Responsibilities
- Conduct initial and ongoing PT assessments for residents in a skilled nursing setting.
- Develop and implement individualized treatment plans to improve mobility, strength, endurance, balance, and functional independence.
- Provide therapeutic exercises, gait training, balance activities, ROM work, and modality-based interventions as prescribed.
- Document patient progress, modify plans of care as needed, and communicate with the interdisciplinary team.
- Educate residents and families on home exercise programs, safety, fall prevention, and activity modification.
- Ensure safety, proper use of equipment, and adherence to regulatory and documentation standards.
Requirements
- Active Physical Therapist license in California (or eligibility to obtain CA licensure).
- Doctor of Physical Therapy (DPT) degree or equivalent.
- Experience in geriatric or skilled nursing facility rehabilitation preferred; ability to work with older adult populations.
- Strong communication and documentation skills; proficient with standard electronic medical record systems.
- Ability to work a 36-hour per week schedule in a skilled nursing facility environment; flexible approach to patient needs.
- Willingness to travel for a travel assignment and adhere to assignment terms.
