Responsibilities
- Provide and document skilled nursing care to patients in home settings, focusing on comfort, symptom management, and rehabilitation goals.
- Implement and update care plans, monitor patient status, and intervene as needed according to protocols and policies.
- Administer medications and treatments, assess responses, and communicate changes to the interdisciplinary team.
- Collaborate with physicians, case managers, and other providers to coordinate and optimize at-home care; maintain accurate clinical records.
- Educate patients and families on disease process, medications, self-care techniques, and safety in the home; promote independence when possible.
- Exercise independent judgment in routine and urgent situations; maintain professional boundaries, confidentiality, and adherence to regulatory guidelines.
- Operate in a home-based setting with the ability to drive to patient homes; respond to emergencies and participate in on-call coverage as required.
Requirements
- Massachusetts Temporary Nursing Service Agency License (Nursing Pool) must be uploaded with traveler profile prior to submission.
- Massachusetts LPN license must be uploaded with profile.
- Active American Heart Association BLS or American Red Cross certification.
- One year of acute nursing care experience required; hospital-based experience preferred; home health experience required.
- Certified Home Health knowledge including proficiency with OASIS highly preferred; infusion therapy experience preferred.
- Ability to work independently in a home health/hospice setting; strong critical thinking and documentation skills.
- Valid driver’s license and access to a reliable vehicle; ability to drive in inclement weather.
- Willingness to be on-call as needed; RTO requests must be noted at submission (no more than 5 days preferred).
