Responsibilities
- Provide direct and indirect skilled nursing care for adult and geriatric residents under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team.
- Contribute to development and ongoing refinement of an age-appropriate care plan, involving residents and families as needed.
- Administer prescribed medications and treatments per orders and facility policy; monitor for adverse effects.
- Collaborate with physicians, therapists, and other care team members to ensure safe, effective, and holistic resident care; participate in continuing education and onboarding of staff as needed.
- Maintain current nursing knowledge and clinical competencies; promote a safe environment and advocate for resident well-being.
Requirements
- Active New York LPN license
- BLS certification (American Heart Association or RQI)
- Minimum 1 year of LPN experience in a long-term care setting
- Ability to perform the essential duties in a geriatric/adult care environment
- Completion of an approved LPN education program (diploma/certificate)
