Responsibilities
- Administer medications and treatments per the resident care plans under supervision of a registered nurse
- Monitor residents’ conditions, document vital signs, changes in status, and response to interventions
- Provide direct patient care and assistance with activities of daily living (bathing, grooming, feeding, mobility)
- Collaborate with licensed staff and interdisciplinary team to update and implement care plans
- Ensure patient safety, maintain infection control practices, and uphold falls prevention protocols
- Document on charting systems accurately and promptly; report changes to the supervising nurse
Requirements
- Active Licensed Practical Nurse license (New Hampshire) or multi-state license as applicable
- Current CPR/BLS certification
- Nursing diploma or equivalent from an approved LPN program
- Minimum prior experience in a long-term care or skilled nursing setting preferred
- Ability to work the listed schedule (Sun, Mon, Tue) and adapt to a fast-paced LTC environment
