Responsibilities
- Perform case management duties focused on medical necessity review, utilization review, and discharge planning.
- Identify and communicate changes in plan of care; coordinate with the care team and community resources.
- Anticipate patient needs using LOS data; document screenings and discharge plans in the patient record (Epic).
- Conduct concurrent/continued stay reviews; participate in appeals/denials as needed; verify DRG/CMS benefits eligibility.
- Complete needs assessment, arrange DME ordering, and ensure HIPAA compliance in all care coordination activities.
- Maintain knowledge of resources and referrals to support transitions across settings for adults, pediatrics, and geriatrics.
Requirements
- Active RN license in Idaho or valid compact license in hand at time of submittal.
- BLS certification.
- Epic charting experience required.
- Minimum 2 years of RN experience preferred; prior charge experience and facility experience in a community hospital are preferred.
- Travel experience not required/unspecified; prior unit experience with case management is leveraged.
- Onboarding labs/core items must be completed within 48 hours of contract signing.
- 100-mile radius rule – local candidates will not be considered.
- Ability to work in a hospital-based setting serving adults, pediatrics, and geriatrics.
