Responsibilities
- Lead discharge planning and care coordination for patients across the hospital stay, ensuring safe transitions to the next level of care
- Assess patient needs, develop and update the plan of care, and coordinate with interdisciplinary teams including Social Services
- Order durable medical equipment (DME) and other community-based resources as part of care transitions
- Maintain accurate, timely documentation in the electronic medical record (EPIC)
- Manage a caseload of approximately 25–30 patients with attention to diverse patient populations (neonates through geriatrics)
- Float across hospital units and campuses as needed to support care continuity and workload balance
Requirements
- Active RN license in Washington or a Compact state (WA RN or Compact accepted)
- BLS certification required
- Minimum 1 year of acute care experience with discharge planning and care coordination
- Proficiency with EPIC or similar EMR systems
-Ability to manage a 25–30 patient caseload; strong organizational and communication skills
- Willingness to float across hospital units and campuses; weekend and holiday coverage as scheduled
- Education: Associate or Bachelor’s degree in Nursing from an accredited program
- Orientation: approximately 30 hours
- Pre-approved time off: 1 period per contract
- Background and references: minimum of 2 supervisory references (within the last 3 years); background/Nursys disclosure at submittal
- COVID-19 vaccine: not required; flu vaccine status not specified
- Locals: not accepted
