Responsibilities
- Deliver coordinated nursing care for an assigned group of patients in a home hospice/ER-related context according to established standards and the nursing process.
- Assess, plan, implement, and evaluate patient care; identify outcomes and adapt care plans as needed.
- Administer medications and treatments, monitor patient responses, and document in the EPIC EMR with accuracy and timeliness.
- Collaborate with an interdisciplinary team (patients, families, physicians, and other providers) to coordinate care, discharge planning, and education.
- Supervise and direct activities of nursing staff as assigned; delegate tasks appropriately and maintain a safe, healing environment.
- Ensure compliance with regulatory and facility safety requirements, including HLFS and ACLS/BLS expectations; provide patient and family education as part of care.
Requirements
- Licensure: Active Registered Nurse (RN) license in the state of practice (CA) is required.
- Certifications: Current BLS or CPR certification; ACLS certification; HLFS (Hospital Fire and Life Safety) compliance as required.
- Experience: At least two years of recent nursing experience in the field; acute care facility experience preferred.
- Education: BSN preferred.
- EMR: EPIC experience required for efficient charting and care coordination.
- Additional: Ability to work in a home healthcare - hospice setting; willingness to travel for a 13-week assignment; ability to work nights.
