Responsibilities
- Provide direct nursing care to adult and elderly residents under supervision of a registered nurse.
- Administer medications and treatments as prescribed, document administration accurately, and monitor for adverse effects.
- Assess residents’ condition, measure vital signs, and report changes promptly to the supervising RN.
- Assist with plan of care development and updates, contributing to ongoing care coordination.
- Support activities of daily living and rehabilitation tasks, promoting resident safety and comfort.
- Maintain accurate charting and documentation in accordance with policy, regulatory requirements, and facility procedures.
- Ensure infection prevention, proper use of equipment, and adherence to safety protocols; respond to call lights in a timely manner.
Requirements
- Active LPN license in New Hampshire (or current eligibility per state requirements)
- Graduation from an approved practical nursing program
- Ability to work night shift (7:00 PM–7:30 AM) and meet 36 hours per week
- Local residency or commutable distance to Laconia, NH
- Basic computer and documentation skills; strong communication and teamwork
- Ability to comply with facility policies and complete standard pre-employment screenings
