Responsibilities
- Deliver coordinated nursing care for a patient or an assigned group of patients according to established standards of care and the nursing process.
- Provide telehealth nursing assessments and triage via the approved platform; coordinate care with on-site team and other disciplines.
- Supervise and direct activities of various levels of nursing staff; delegate tasks while applying critical thinking and independent judgment.
- Administer medications and treatments per plan; monitor patient status and escalate changes as needed.
- Document care accurately in the patient record; communicate plan of care to patients, families, and the interdisciplinary team.
- Maintain patient safety, infection control, and compliance with policies; participate in care planning and quality improvement efforts.
Requirements
- Active Registered Nurse license in the state of Oregon.
- Ability to work in a telehealth/home health/hospice setting and coordinate with remote and on-site teams.
- Availability for 12-hour shifts; schedule aligns with 3 days per week (36 hours).
- Strong clinical assessment, critical thinking, and communication skills.
- Proficiency with electronic health records and telehealth platforms.
