Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions, while meeting department objectives
- Serve as co-leader in an interdisciplinary approach to patient groups, collaborating with team members across disciplines
- Assess patient status, document progress, and adjust treatment plans to optimize functional outcomes
- Deliver evidence-based physical therapy interventions tailored to home-based care
- Communicate patient progress and barriers with the care team to support coordinated care
- Maintain compliant documentation and ensure adherence to department policies and standards
Requirements
- Active physical therapist license in California
- Doctor of Physical Therapy degree or eligibility
- Demonstrated ability to work effectively within an interdisciplinary team
- Strong communication, assessment, and documentation skills
- Ability to travel for a 13-week assignment and work in a home health setting
- Commitment to patient-centered care and adherence to clinical guidelines
