Responsibilities
- Administer medications and treatments per plan under supervision
- Monitor patient condition, vital signs, and respond to changes promptly
- Provide routine nursing care and assist with activities of daily living
- Document care provided, maintain accurate charts in the electronic health record
- Assist in care planning with the supervising RN and communicate with patients and families
- Ensure infection control, safety, and adherence to healthcare regulations
Requirements
- Current LPN licensure
- Ability to work evenings and adapt to a long-term care setting
- Willingness to travel for a short-term assignment
- Strong communication and documentation skills
- Ability to follow care plans and collaborate with the healthcare team
