Responsibilities
- Deliver coordinated nursing care for an assigned group of patients according to established standards of care and the nursing process.
- Assess, plan, implement, and evaluate patient care; monitor patient status and respond to changes with clinical judgment.
- Administer prescribed medications and therapies; observe for adverse effects and document responses.
- Supervise and direct activities of various nursing staff levels; coordinate care with other disciplines to ensure integrated treatment plans.
- Maintain patient safety, infection control, and accurate documentation; communicate care plans and progress with patients, families, and the healthcare team.
Requirements
- Active California Registered Nurse license
- Current CPR/BLS certification
- ER nursing experience or related acute care background
- Ability to provide compassionate, patient-centered care in a home hospice setting
- Eligibility to work a travel assignment in California and to perform 12-hour day shifts (36 hours/week)
- Strong communication and documentation skills
