Responsibilities
- Perform comprehensive PT evaluations for residents and develop individualized treatment plans aligned with goals and prognosis
- Deliver evidence-based interventions to improve mobility, strength, balance, gait, and functional independence
- Monitor progress, adjust plans as needed, and document outcomes and care in the chart
- Reassess residents regularly and coordinate with the interdisciplinary team to optimize care planning
- Educate residents and families on home exercise programs, safety strategies, and discharge planning
- Maintain compliant documentation and adhere to regulatory requirements and facility policies

Requirements
- Active Maryland Physical Therapist license or eligibility for licensure in Maryland
- Doctor of Physical Therapy (DPT) or equivalent clinical degree
- 1+ year of experience in skilled nursing facility or geriatric rehabilitation preferred
- Ability to work effectively in a skilled nursing facility setting
- Strong communication skills and ability to collaborate with an interdisciplinary team
