Registered Nurse – Case Manager (RN or MSW) in Everett, WA. Start date: 06/01/2026. Contract length: 8 weeks. Weekly pay: 2,048 to 2,126 per week. Shift: 4x10 days (days). This travel temporary assignment takes place in a hospital setting with a Case Management / Care Coordination focus, including discharge planning and care coordination for patients across ages. Guaranteed hours: 72 hours per 2-week period.

Responsibilities
- Lead discharge planning and care coordination for inpatient populations, developing and updating plans of care across service lines.
- Conduct needs assessments, arrange post-acute services, DME orders, and coordinate transitions to the next level of care; collaborate with Social Services and the interdisciplinary team.
- Work across hospital areas (Med-Surg, ED, and other service lines as needed) to ensure timely patient throughput and smooth transitions.
- Document thoroughly in the electronic charting system (EPIC) and maintain accurate, up-to-date records of assessments, plans, and outcomes.
- Maintain an expected patient load consistent with unit norms (approximately 25–30 cases) and participate in daily rounds and care coordination workflows.
- Communicate clearly with patients, families, and other care team members; provide education on discharge plans and post-discharge care.

Requirements
- Licensure: RN licensure required for RN applicants (MSW applicants accepted per role).
- Education: RN degree or MSW degree as applicable to the role.
- Certification: BLS required.
- Experience: Minimum 1 year of acute care experience; discharge planning / care coordination experience required.
- Skills: Strong clinical foundation in acute hospital care, discharge planning, and care coordination; knowledge of needs assessment and DME ordering; ability to develop and monitor plans of care.
- Charting: EPIC experience required.
- Additional: Must be able to work day shifts; willing to float to all hospital areas as needed; strong preference for diverse case management experience (Med-Surg, LLOS, ED); two references covering 1 year within the last 3 years.
- Licensing logistics: WA RN or Compact RN licensure required for RN applicants.
