Responsibilities
- Evaluate patients and develop individualized OT treatment plans to improve daily living and work skills, promote independence, and enhance safety.
- Implement therapeutic interventions and monitor patient progress within an outpatient/urgent care setting.
- Collaborate with an interdisciplinary care team to align OT goals with overall care plans and coordinate care for geriatric populations.
- Document assessments, goals, progress notes, and discharge planning in accordance with regulatory standards and facility policies.
- Educate patients and families on home exercise programs, adaptive equipment, safety strategies, and fall prevention.
- Ensure compliance with licensure requirements, professional ethics, and best-practice guidelines.
Requirements
- Active Colorado Occupational Therapy license and NBCOT registration
- Education: Bachelor's, Master's, or Doctorate in Occupational Therapy from an accredited program
- Experience: 1–3 years as an Occupational Therapist preferred; at least 1 year working with elderly and frail populations
- Valid driver's license with reliable transportation
- Ability to work in an outpatient/urgent care setting in Grand Junction, CO
- Willingness to participate in a travel, temporary assignment and adhere to shift schedule
