This 13-week travel assignment is for an Occupational Therapist in an outpatient urgent care setting in Grand Junction, CO. The role focuses on evaluating and implementing individualized therapy to help patients develop, recover, or maintain daily living and work skills, with emphasis on promoting independence, safety, and quality of life. The assignment may involve coordination within an interdisciplinary care plan and participation in geriatric care programs as indicated by patient needs.

Responsibilities
- Evaluate patients and develop individualized OT treatment plans to improve or maintain daily living and work activities.
- Implement therapeutic interventions aligned with interdisciplinary care plans; support geriatric and frail populations as indicated.
- Reassess progress, document outcomes, and adjust plans to optimize independence and safety.
- Educate patients and caregivers on home programs, activity modification, and safety strategies to support treatment goals.
- Maintain adherence to documentation standards, safety protocols, and infection control practices.
- Collaborate with the care team to ensure coordinated, patient-centered care and timely communication of patient progress.
Requirements
- Active Colorado occupational therapy license and NBCOT certification
- Degree in Occupational Therapy (Bachelor’s, Master’s, or Doctorate) from an accredited program
- 1–3 years of OT experience preferred; 1 year working with elderly/frail populations required
- Ability to work in an outpatient, urgent care setting; Monday–Friday, 8:00 am–5:00 pm
- Valid driver’s license with reliable transportation
