Responsibilities
- Provide direct nursing care to residents in a long-term care setting, including monitoring vital signs, medication administration, wound care, and routine assessments.
- Develop and implement care plans in collaboration with the interdisciplinary team; document changes and ensure accurate charting.
- Administer prescribed medications and treatments, adhering to medication administration records and safety protocols.
- Support residents with activities of daily living, mobility, and comfort, while promoting dignity and independence.
- Monitor residents for changes in condition, respond to acute events, and communicate findings promptly to the supervising nurse or physician.
- Maintain infection control practices, safety standards, and patient rights; document care activities accurately.
Requirements
- Active LPN license in good standing (state not specified)
- Experience in long-term care or geriatric nursing preferred
- Ability to perform 12-hour shifts and maintain accurate documentation in electronic health records
- Strong assessment, observation, and communication skills; teamwork-oriented
- Comfortable working in a long-term care setting and following facility policies
