Responsibilities
- Provide routine and specialized nursing care to residents per physician orders and care plans
- Administer medications, treatments, and injections as prescribed; monitor for adverse effects
- Assess residents’ conditions, document findings, and communicate changes to the care team
- Perform wound care, ADL assistance, and routine nursing tasks while maintaining patient safety
- Collaborate with physicians, therapists, and other staff to develop and adjust individualized care plans
- Document care activities accurately in the electronic health record and ensure compliance with policies and regulatory standards
Requirements
- Active LPN license in good standing
- Ability to work on-site in the listed location and participate in the 3x12-hour shift schedule
- Strong communication and teamwork skills; ability to interact compassionately with residents and families
- Proficiency with basic computer charting and health record documentation
- Local candidates preferred for this assignment (local only)
