Responsibilities
- Provide RN case management for home health patients, including assessment, care planning, and coordination with the care team.
- Conduct comprehensive patient assessments and OASIS data collection as part of routine care documentation.
- Manage a relatively high-volume caseload, approximately 5–6 patient visits per day, with thorough and timely documentation.
- Administer clinical care and wound care as needed, adapting plans based on patient status and interdisciplinary input.
- Ensure regulatory and quality standards are met, maintain accurate charting, and communicate effectively with physicians, therapists, and family caregivers.
- Educate patients and families on disease management, medications, safety, and discharge planning to support transitions of care.
Requirements
- Active RN license (state of licensure appropriate for the assignment)
- 2+ years of home health experience
- Case management experience and OASIS familiarity
- Wound care experience preferred
- Ability to manage a high-volume caseload (approx. 5–6 patients per day) with thorough documentation
- Willingness to participate in a travel-focused assignment and work within a home health setting
