Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical function, with attention to social and mental aspects as needed
- Perform comprehensive evaluations and develop individualized treatment plans; implement therapy interventions using established protocols and techniques
- Provide therapy interventions and adjust plans based on patient response; collaborate with a multidisciplinary team and support patient and family education
- Establish and communicate care and discharge plans; ensure comprehensive clinical documentation within established deadlines
- Supervise assigned assistants, maintain patient safety, and contribute to quality improvement initiatives
- Maintain proficiency with EPIC documentation and coordinate care across outpatient and inpatient settings

Requirements
- Doctor of Physical Therapy (DPT) degree; required when license was obtained on or after January 2016, or foreign equivalent
- Current Physical Therapy licensure in the state of practice
- CPR certification or Basic Life Support (BLS) certification from an approved vendor per policy
- At least two years of recent Physical Therapy experience preferred
- Current nursing license per the state of assignment
- EPIC experience required
- Experience in outpatient and inpatient settings preferred
- Ability to work in a hospital-based setting and meet assignment requirements
