Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial condition and communicate findings to the RN and Interdisciplinary Team.
- Contribute to development of an age-appropriate, culturally relevant plan of care reflecting nursing diagnoses and interventions, involving residents and families.
- Recommend revisions to the plan of care after evaluating resident outcomes and coordinate referrals as needed.
- Maintain current knowledge and competency in nursing practice; participate in mentoring/orientation of staff; provide leadership within the professional practice setting.
- Follow policies and procedures, ensure safety and infection control, and maintain collaboration with physicians, nursing staff, and ancillary personnel.
- Adhere to safety protocols, including occupational exposure considerations related to bloodborne pathogens.
Requirements
- Active New York Licensed Practical Nurse (LPN) license
- BLS certification (American Heart Association or RQI accepted)
- Minimum 1 year of experience as an LPN in a long-term care facility
- Demonstrated ability to provide care for adult and geriatric residents
- Ability to work in a long-term care setting and collaborate with an interdisciplinary team
