Responsibilities
- Conduct comprehensive PT evaluations and establish individualized treatment plans to restore and improve function
- Deliver outpatient PT interventions using standard protocols and adjust plans based on patient response
- Collaborate as a co-leader within an interdisciplinary team and contribute to group-based patient care
- Develop, communicate, and implement discharge plans; ensure accurate documentation and timely communication with the care team
- Complete comprehensive clinical documentation in a timely manner; supervise assigned assistants as needed
- Maintain patient safety and adherence to clinical policies; support EPIC-based charting and data entry
Requirements
- Licenses: Physical Therapy licensure in the state of practice (California) is required
- Education: Doctor of Physical Therapy (DPT) degree; if license was obtained on or after January 2016, the DPT is required or foreign equivalent
- Experience: Two years of physical therapy experience is preferred
- Certifications: CPR or Basic Life Support (BLS) certification from an approved vendor
- Additional: Must have EPIC experience; ability to work in an outpatient setting; no on-call requirements
- Start date and schedule compatibility with 13-week travel assignment; ability to work the listed shift pattern
