Responsibilities
- Provide direct and indirect skilled nursing care to adult and geriatric residents under RN supervision.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team.
- Contribute to development and updates of an age-appropriate, culturally relevant plan of care involving the resident, family, and care team.
- Administer medications and treatments as ordered, monitor responses, and report changes promptly.
- Collaborate with physicians, RNs, and ancillary staff to maintain a safe and effective care environment; participate in education and safety initiatives.
Requirements
- Active New York LPN license
- BLS certification (AHA or RQI accepted)
- Minimum 1 year of LPN experience in a long-term care setting
- Ability to work nights; weekends and holidays as required
- Demonstrated knowledge of adult and geriatric care; ability to function in a fast-paced skilled nursing environment
