Responsibilities
- Evaluate residents, determine rehabilitation needs, and develop individualized plans of care in collaboration with the interdisciplinary team.
- Implement therapeutic interventions to improve mobility, strength, balance, and functional independence; monitor progress and adjust plans as needed.
- Document goals, treatment notes, and progress in accordance with regulatory and facility requirements.
- Educate patients and families on treatment plans, home exercise programs, and safety strategies to support continued recovery.
- Ensure patient safety during therapy sessions and maintain equipment and treatment areas in good working order.
- Communicate timely clinical information to supervisors and team members to support coordinated care.
Requirements
- Current physical therapy licensure to practice in the United States (NM licensure or eligibility preferred).
- Experience in a skilled nursing facility or post-acute rehabilitation setting is preferred.
- Ability to commit to 35 guaranteed hours per week.
- Availability for a 08:00–16:30 schedule, five days per week, in Albuquerque, NM.
- Strong documentation, communication, and collaboration skills to support interdisciplinary care.
