Responsibilities
- Assess patient needs and coordinate case management plans to optimize care transitions and outcomes.
- Develop and implement discharge planning, care pathways, and resource utilization with interdisciplinary teams.
- Communicate with patients, families, and healthcare providers to support care decisions and education.
- Document case management activities accurately, ensuring compliance with regulatory and hospital policies.
- Facilitate referrals to community resources, payors, and ancillary services as appropriate.
- Adapt duties to support unit operations, including potential floating to sister campuses as needed.
Requirements
- Active Florida RN license (FL/Compact) with recent Nursys on profile.
- Degree accreditation must be on profile or the candidate must have 5 years of experience.
- 2 years of Case Manager experience required; acute care experience required.
- BLS certification required.
- Prior travel experience required.
- Ability to meet a 50-mile radius rule from permanent address.
- Floating required; travelers are first to float and may cover other campuses as needed.
- Demographic information (address, email, full DOB, full SSN, RN license number) must be loaded at submission for profile verification.
