Responsibilities
- Provide direct patient care to residents in a skilled nursing setting, following the established plan of care.
- Administer prescribed medications and monitor patient responses; recognize and report adverse effects.
- Conduct comprehensive assessments, contribute to care planning, and update records to reflect changes in condition.
- Monitor vital signs, assist with activities of daily living, wound care, and routine treatments while prioritizing patient safety.
- Collaborate with interdisciplinary team members (physicians, therapists, social workers) to coordinate care and communicate changes promptly.
- Maintain accurate documentation in the electronic health record; assist with admissions, transfers, and discharges as needed.
Requirements
- Active Registered Nurse license in Colorado
- Current CPR/BLS certification
- ADN or higher nursing degree
- 1+ year of RN experience in skilled nursing facility or geriatric care preferred
- Ability to work in a skilled nursing setting and adhere to facility policies
- Strong communication, critical thinking, and teamwork skills
