Responsibilities
- Collect data relevant to residents’ physical and psychosocial condition and communicate findings to the RN and Interdisciplinary Team.
- Contribute to a developmentally appropriate, culturally relevant plan of resident care reflecting nursing diagnoses and interventions to achieve outcomes; involve the resident/family and healthcare team.
- Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals to ensure all aspects of care are addressed; collaborate with resident/family and team.
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse; maintain knowledge and clinical expertise through continuing education.
- Maintain current knowledge and competency in nursing practice; participate in mentoring/orientation of staff; consider safety, cost, and impact in care delivery; provide leadership in the professional practice setting as directed.
- Be aware that occupational exposure to bloodborne pathogens is reasonably anticipated; tasks may involve contact with blood, body fluids, or tissues.
Requirements
- Active NY LPN license
- BLS certification (American Heart Association or RQI)
- Minimum 1 year of LPN experience in a long-term care facility
- Must be able to start on a Tuesday or Thursday for full-shift orientation; LPN Full Time Days/Evens rotation (5x8s)
- Weekend and holiday coverage may be required
- Ability to provide care to adult and geriatric residents
