Responsibilities
- Deliver direct skilled nursing care to hospice patients in the home setting in accordance with established standards of care and the nursing process.
- Assess patient needs, develop and implement plans of care, monitor outcomes, and adjust interventions as needed to support comfort and safety.
- Supervise and direct activities of nursing staff at various levels, coordinating with the broader care team to ensure comprehensive patient management.
- Coordinate care with physicians, social workers, and other disciplines; exercise critical thinking, professional judgment, and independent decision-making.
- Document assessments, plans, interventions, and patient/family education accurately in the medical record; communicate changes promptly to the team and family.
Requirements
- Active California RN license
- Oncology or hospice nursing experience preferred
- Ability to provide care in a home health/hospice setting; strong assessment and communication skills
- Willingness to work 3x12-hour day shifts for a 13-week assignment; reliable transportation to patient locations as needed
- Strong collaboration with multidisciplinary teams and ability to function in a fast-paced, autonomous role
- Willingness to complete credentialing and onboarding requirements for a travel assignment
