Responsibilities
- Deliver coordinated nursing care for a patient or assigned group according to established standards and the nursing process.
- Collect relevant data, assess, analyze, and develop a care plan; implement and evaluate progress with interdisciplinary collaboration.
- Supervise and direct activities of various levels of nursing staff; delegate duties and coordinate care with other disciplines.
- Participate in discharge planning to ensure continuity of care and patient safety in the home healthcare environment.
- Educate patients and family members, provide health information, and promote a safe and healing environment.
- Document care and patient information accurately in the electronic health record (EPIC); ensure clear communication with the treatment team.
Requirements
- Active Registered Nurse (RN) license in the state of California (per state of assignment)
- BLS/CPR certification from an approved vendor per AH policy; HLFS (Hospital Fire and Life Safety) required
- ACLS certification required; PALS certification required
- NIH Stroke Scale (NIHSS) certification required
- EPIC EMR experience required
- Minimum of two years of recent RN experience in the field; acute care exposure preferred
- BSN degree preferred
