Responsibilities
- Administer prescribed medications and treatments under supervision, ensuring accuracy and safety
- Monitor resident conditions, perform routine vital signs, and document findings in the chart
- Assist residents with activities of daily living, provide mobility support, and promote functional independence
- Observe for changes in status, report promptly to the supervising RN, and collaborate with the interdisciplinary care team
- Maintain a safe and clean environment, follow infection control practices, and uphold resident dignity and privacy
- Complete timely and accurate charting, including updates to care plans and daily progress notes
Requirements
- Active Licensed Practical Nurse license
- Completion of an approved LPN educational program
- Ability to work night shifts in a long-term care setting
- Experience in long-term care or skilled nursing preferred
- Strong communication and teamwork skills; ability to document clearly and concisely
