Responsibilities
- Conduct initial physical therapy evaluations and develop individualized treatment plans for residents in a long-term care setting
- Implement PT interventions, including therapeutic exercises, gait training, balance activities, and modalities as appropriate
- Reassess progress, document outcomes, and adjust plans as needed; communicate changes to the interdisciplinary team
- Collaborate with physicians, nurses, and ancillary staff; educate residents and families on therapy plans and goals
- Ensure compliance with regulatory requirements and facility policies; maintain accurate documentation in the electronic medical record
- Maintain safety and infection control standards; use proper patient handling and safety procedures during therapy sessions
Requirements
- Current Physical Therapist license in the state of Washington or eligibility for licensure
- 1+ year of PT experience in a skilled nursing or long-term care setting preferred
- Ability to work the listed shift and guaranteed hours (8:00 AM–4:30 PM, 35 hours/week)
- Strong communication and documentation skills; ability to work effectively in a team-based environment
