Physical Therapist – Skilled Nursing Facility in Rochester, New Hampshire. Travel, temporary assignment beginning 06/29/2026 for 14 weeks. Guaranteed hours: 35 per week. Weekly pay range: $2,201–$2,364 per week. Shift: 5x8 Days (08:00–16:30). This position occurs in a long-term care setting focused on geriatric rehabilitation.

Responsibilities
- Conduct comprehensive PT evaluations for residents in the skilled nursing environment
- Develop and implement individualized treatment plans to improve mobility, balance, strength, and functional independence
- Administer therapy interventions, including therapeutic exercises, gait training, transfers, and modalities as appropriate
- Monitor progress, adjust treatment plans, and document outcomes in a timely, compliant manner
- Collaborate with an interdisciplinary team to support discharge planning and optimize resident outcomes
- Educate residents and families on home programs, safety strategies, and fall prevention
Requirements
- Active physical therapy license with eligibility to practice in the United States; NH licensure or eligibility is preferable
- Experience in geriatric rehabilitation or skilled nursing facility settings preferred
- Ability to commit to guaranteed 35 hours per week with a daytime 08:00–16:30 shift
- Strong documentation, communication, and collaboration skills
- Comfortable working in a fast-paced, multi-disciplinary care environment
- Eligible for a 14-week travel assignment
