Responsibilities
- Conduct comprehensive occupational therapy evaluations for home health patients and develop individualized plans of care.
- Implement evidence-based OT interventions to improve activities of daily living, motor and cognitive skills, and functional independence.
- Reassess progress regularly and adjust treatment plans to meet patient goals and clinical outcomes.
- Educate patients and families on activities of daily living, safety strategies, adaptive equipment, energy conservation, and home modification needs.
- Document assessments, goals, treatment sessions, progress notes, and outcomes in the patient record; communicate with the interdisciplinary team to coordinate care.
- Maintain compliance with licensing, documentation, and safety requirements; remain current with professional standards and applicable regulations.

Requirements
- Active Occupational Therapy license in applicable states of practice (Oregon licensure required; multi-state licensure may be needed).
- Master’s degree in Occupational Therapy or Doctor of Occupational Therapy (OTD): Required; only if license obtained on or after January 2007; or foreign equivalent.
- CPR certification or Basic Life Support (BLS) certification from an approved vendor per policy: Required.
- Valid Driver’s License and must be at least 21 years of age: Required.
- Epic experience preferred; provide details if available.
- Experience: OT practice experience required; home health experience is a plus.
- Ability to work in a home health setting and travel to patient locations as part of the assignment.
