Responsibilities
- Evaluate and treat patients requiring occupational therapy in acute rehab and long-term care settings.
- Develop and implement individualized treatment plans to improve activities of daily living, mobility, and functional skills.
- Administer therapeutic interventions, monitor progress, and adjust plans based on patient response.
- Document evaluations, progress notes, and discharge planning results in the medical record, following professional and regulatory standards.
- Collaborate with an interdisciplinary team (e.g., physical therapy, speech-language pathology, nursing, social work) to coordinate care and optimize outcomes.
- Provide patient and family education, including home exercise programs and community resources, and participate in discharge planning to support successful transitions.
