Responsibilities
- Conduct comprehensive speech-language pathology evaluations, including functional assessment and, when appropriate, home environment considerations; identify equipment needs relative to speech-language function.
- Develop and implement a Plan of Care (POC) to improve communication, swallowing, and related functional outcomes; administer therapy using evidence-based techniques and/ or assistive devices.
- Recommend communication devices or aids as indicated; collaborate with the referring physician and interdisciplinary team to ensure coordinated, comprehensive care.
- Document patient progress, treatment effectiveness, and continuing need for therapy; provide education to patients and families to support continued functional independence.
- Identify referrals to additional services as appropriate; participate in case conferences and professional development activities to support care planning.
- May supervise home health aides in procedures directly related to the speech-language POC; engage in ongoing professional growth and maintain clinical competency.

Requirements
- Master’s degree in Speech-Language Pathology from an accredited program (ASHA standards) or coursework/work experience that meets ASHA certification requirements
- Current Certificate of Clinical Competence (CCC) in Speech-Language Pathology from ASHA, or CCC eligible and hold current state licensure as applicable
- Current CPR certification
- Current valid driver’s license and reliable transportation with adequate auto insurance
- Must read, write, and speak fluent English
- 1 year of clinical experience in Speech-Language Pathology required
- Experience in a home care environment preferred
- Ability to work in a community-based care setting and travel locally as needed
- Must be able to perform essential job duties with or without reasonable accommodation
