Responsibilities
- Provide direct nursing care to residents in a long-term care setting, including monitoring health status and responding to patient needs.
- Administer medications and treatments as prescribed, verify orders, and document administration accurately.
- Monitor vital signs, observe for changes in condition, and report findings to the supervising nurse or physician.
- Document assessments, care plans, and interventions in the PointClickCare EMR system; update as resident status evolves.
- Assist residents with activities of daily living (ADLs), mobility, and personal care; support safety and fall prevention.
- Collaborate with the interdisciplinary team to implement care plans and participate in rounds and care conferences as assigned.
- Maintain infection control and safety protocols; respond to emergencies and initiate appropriate escalation procedures.

Requirements
- Active New York Licensed Practical Nurse (LPN) license; Nursys verification of active license.
- Current Basic Life Support (BLS) certification.
- Minimum of 1 year of experience in a long-term care (LTC) facility.
- EMR experience with PointClickCare.
- Driver’s license with address matching demographics.
- RTO (time off) request must be included at the time of submission.
