Responsibilities
- Perform comprehensive evaluations of communication disorders, cognition, voice, fluency, and swallowing for residents in a skilled nursing setting.
- Develop, implement, and adjust individualized treatment plans; deliver direct therapy sessions and monitor changes in function and participation.
- Document all assessments, treatment notes, progress measures, and discharge summaries in the electronic health record while adhering to regulatory and facility standards.
- Collaborate daily with an interdisciplinary team (nurses, therapists, physicians, social work) to coordinate care and share updates with patients and families.
- Reassess goals regularly, modify therapy as needed, and participate in discharge planning and transition planning for residents returning to community or long-term care.
- Maintain patient and family education about therapy goals, expectations, and home strategies; ensure safety, consent, and privacy in all interactions.
Requirements
- Active California license to practice Speech-Language Pathology or clear eligibility to obtain licensure.
- Master’s degree in Speech-Language Pathology from an accredited program.
- Certificate of Clinical Competence (CCC-SLP) from ASHA or CFY eligibility.
- Demonstrated ability to work effectively in a skilled nursing setting with older adults; solid documentation skills and comfort with EHRs.
- Willingness and ability to participate in a travel assignment and adapt to new teams, facilities, and patient populations.
- Strong communication, organization, and time-management skills; ability to collaborate within an interprofessional team.
