Responsibilities
- Provide direct, compassionate nursing care to residents in a long-term care setting, following individualized care plans and facility policy.
- Administer prescribed medications and treatments within the scope of LPN practice; monitor responses and document outcomes.
- Perform routine assessments, monitor vital signs, wound care, and early recognition of clinical changes; escalate to the supervising RN or physician as appropriate.
- Maintain accurate and timely documentation in the resident’s medical record; communicate changes to the care team clearly and promptly.
- Collaborate with RNs, CNAs, and other members of the interdisciplinary team to support safety, comfort, and quality of life for residents.
- Assist residents with activities of daily living as needed, while promoting independence and dignity.
Requirements
- Active Licensed Practical Nurse (LPN) license in New Hampshire, or eligibility to practice per state requirements.
- Experience in a long-term care or skilled nursing setting preferred.
- Ability to work the evenings schedule and fulfill the every-other-weekend requirement.
- Willingness to participate in a 13-week travel assignment with guaranteed hours.
