Responsibilities
- Provide direct patient care and support under the supervision of registered nurses and physicians in a long-term care and acute rehab environment
- Monitor vital signs, administer prescribed medications and treatments, and document patient responses accurately
- Assist with activities of daily living, mobility, and rehabilitation activities as directed by the care team
- Observe and report changes in patient condition, including pain, confusion, or functional status, to the supervising nurse
- Maintain safe, clean, and therapeutic patient environments; adhere to infection control and safety protocols
- Collaborate with an interdisciplinary team (nursing, therapy, social work) to support care plans and discharge planning as appropriate
