Responsibilities
- Plan and conduct segments of physical therapy treatment programs designed to restore and improve physical, social, and mental functions, while meeting department objectives.
- Serve as co-leader in an interdisciplinary approach to patient groups, collaborating with nurses, social workers, and other clinicians.
- Deliver therapy services in the home setting according to the established plan of care and adjust interventions as needed.
- Assess patient status, document progress, and communicate changes to the care team; ensure records reflect current goals and outcomes.
- Educate patients and caregivers on exercises, activity modification, and strategies to maintain function and safety in daily activities.
Requirements
- Current active Physical Therapist license in California (DPT or equivalent required/strongly preferred).
- Doctor of Physical Therapy degree (or equivalent) from an accredited program.
- Ability to provide services in a home health hospice setting and travel within the Marysville area.
- Strong communication skills and ability to work effectively within an interdisciplinary team.
- Prior experience delivering physical therapy in home-based or hospice care is preferred.
