Responsibilities
- Administer prescribed medications and document administration accurately in the charting system
- Monitor residents’ conditions, perform routine assessments, and report changes to the interdisciplinary team
- Provide direct nursing care, including wound care, catheter care, injections, and vital sign monitoring
- Assist residents with activities of daily living (ADLs) and ensure comfort, safety, and dignity
- Collaborate with RNs, physicians, and support staff; maintain clear, timely electronic health records
- Adhere to infection control practices, safety protocols, and regulatory requirements
Requirements
- Active Licensed Practical Nurse (LPN) license in Pennsylvania
- Current Basic Life Support (BLS) certification
- Experience in long-term care or geriatric care preferred
- Ability to work evening and night shifts
- Strong communication, teamwork, and documentation skills
- Ability to pass standard pre-employment screenings as part of travel assignments
