- Collaborate with the interdisciplinary team to coordinate and delegate patient care, ensuring safe and effective handoffs
- Adhere to established Standards of Nursing Practice and institutional policies; promote cooperative teamwork and patient safety
- Document assessments, treatments, and responses accurately; communicate clearly with patients, families, and team members
- Support continuity of care through effective discharge planning and patient education as appropriate
