Responsibilities
- Provide licensed practical nursing care to residents in a long-term care setting, following the plan of care and physician orders.
- Administer prescribed medications and treatments, monitor patient responses, and report changes to the supervising RN.
- Monitor residents with dementia, maintain patient safety, manage call-light responses, and document care accurately in the charting system.
- Collaborate with the interdisciplinary team (RN, CNAs, therapy, social work) to coordinate care and adjust plans as needed.
- May float to other units to meet unit needs; ensure continuity of care and adherence to unit policies during transitions.
Requirements
- Active Licensed Practical Nurse (LPN) license in Vermont (or applicable state) in good standing.
- BLS through American Heart Association (AHA) required.
- Minimum 2 years of nursing experience; LTC experience preferred; prior travel experience preferred.
- Comfortable working with residents who have dementia; ability to manage behavioral and cognitive challenges.
- Willingness to float to other units as needed; ability to work a schedule that may vary to meet patient care needs.
- Public transportation is not available in the area; reliable transportation (personal or rental) is strongly recommended.
- All employment gaps over 90 days should be explained during submission; complete work history information (bed count, EMR, facility type, etc.) is required.
