Responsibilities
- Provide direct patient care to residents in a long-term care setting, including routine assessments, medication administration, and monitoring for changes in condition.
- Collaborate with an interdisciplinary team to develop, implement, and update individualized care plans.
- Monitor and document vitals, symptoms, and responses to treatments; recognize and escalate urgent issues promptly.
- Assist residents with activities of daily living; support mobility, transfers, and fall-prevention strategies.
- Administer medications and treatments according to physician orders and facility policies; ensure accurate documentation and reconciliation.
- Maintain a safe, compassionate, and respectful environment; uphold resident rights and infection control practices.
Requirements
- Active Licensed Practical Nurse license in New Mexico (or eligibility/portability to NM) and in good standing.
- Education: Graduation from an accredited practical nursing program.
- Experience: Prior experience in long-term care or geriatric nursing is preferred.
- Skills: Proficiency with nursing assessments, medication administration, and electronic health records; strong communication and teamwork skills.
- Availability to work night shifts and participate in a travel assignment within a long-term care setting.
