Responsibilities
- Deliver case management nursing services for assigned patient population across the continuum of care.
- Perform comprehensive assessments to identify discharge planning needs and care coordination requirements.
- Develop, implement, and monitor individualized care plans in collaboration with interdisciplinary teams.
- Coordinate patient transitions across care settings and with community resources, ensuring smooth handoffs.
- Document care plans, progress notes, and outcomes in the electronic health record; maintain compliance with regulatory standards.
- Communicate effectively with patients, families, and the health care team to support safe, efficient, and quality care.
Requirements
- Active Registered Nurse license in Oregon (in good standing).
- Experience in case management or care coordination preferred.
- Strong communication, collaboration, and documentation skills.
- Ability to perform duties in a fast-paced clinical setting and manage a 40-hour weekly schedule.
- Eligibility for a travel assignment and ability to meet onboarding and credentialing requirements.
- Knowledge of discharge planning processes and resource utilization is helpful.
