Responsibilities
- Assess patients in an acute care setting and develop, implement, and adjust care plans focused on care management and outcomes.
- Coordinate discharge planning, including referrals, transitions of care, and coordination with social work and case management.
- Monitor patient progress and document changes to the plan of care in the electronic health record (Cerner).
- Communicate clearly with patients, families, and the health care team regarding goals of care, utilization, and expected outcomes.
- Collaborate with physicians, nursing staff, and interdisciplinary teams to optimize care delivery and prevent unnecessary readmissions.
- Ensure compliance with regulatory and internal policies while maintaining safe, patient-centered care.

Requirements
- Active RN license in California (state of practice)
- Minimum 1 year acute care management experience
- Cerner experience required
- Case management certification preferred
- Ability to work in a fast-paced acute care environment and participate in a 4x10-hour schedule
- Eligibility to participate in a travel assignment and meet facility onboarding requirements
