Responsibilities
- Administer medications and treatments as prescribed under supervision and document administration correctly.
- Monitor residents’ vital signs, condition changes, and respond to acute needs; escalate as appropriate.
- Assist with activities of daily living and mobility; provide comfort measures and patient-centered care.
- Document care activities, assessments, and observations in the patient record according to facility policy.
- Support the care plan development and participate in interdisciplinary rounds or care conferences as needed.
- Promote safety, infection control, and quality standards; report and address any safety concerns promptly.
Requirements
- Active LPN license (New Mexico) or eligibility to practice in New Mexico.
- Ability to work 18:00–06:30 night shifts on a 3x12-hour schedule with guaranteed hours.
- Prior experience in a skilled nursing facility or long-term care setting preferred.
- Graduate of an accredited practical nursing program.
- Strong communication, teamwork, and documentation skills; ability to work with a diverse resident population.
