Responsibilities
- Provide direct nursing care to residents in a long-term care setting, under the supervision of a registered nurse
- Administer prescribed medications and document administration accurately
- Monitor vital signs and observe for changes in residents’ conditions; report to the supervising clinician
- Assist residents with activities of daily living and maintain a safe, clean, and compassionate environment
- Complete accurate charting and documentation in the electronic medical record; participate in care planning
- Collaborate with interdisciplinary team to implement and adjust care plans; ensure adherence to safety and infection control protocols
Requirements
- Education: Graduate of an approved practical nursing program
- Licensure: Active LPN license in good standing (state-appropriate for assignment)
- Experience: Prior long-term care or skilled nursing experience preferred
- Skills: Strong assessment, documentation, and communication abilities; ability to work collaboratively in a team
- Availability: Ability to work 12-hour shifts and meet guaranteed hours in a local setting
