Responsibilities
- Perform initial and ongoing physical therapy evaluations for residents in a skilled setting
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence
- Deliver evidence-based therapy interventions and adjust plans based on patient progress
- Document outcomes and communicate progress with the interdisciplinary care team
- Educate patients and families on exercises, safety, and home exercise programs; participate in discharge planning as appropriate

- Ability to work in a skilled nursing facility setting
- Strong clinical judgment and documentation skills
- Effective communication and collaboration with a multidisciplinary care team
- Eligibility to work in the United States
- Availability to commit to a 13-week travel assignment with 36 guaranteed hours per week
- Flexibility to work the 08:00–16:30 schedule as required
