Responsibilities
- Conduct initial and ongoing patient assessments to support case management needs
- Develop, implement, and monitor individualized care plans in collaboration with the interdisciplinary team
- Coordinate care transitions, including discharge planning and referrals to appropriate services
- Facilitate resource use to meet patient needs and optimize outcomes
- Document care activities accurately in the medical record and verify adherence to policies and guidelines
- Communicate clearly with patients, families, and team members to align goals and expectations
Requirements
- Active Registered Nurse license (stateful licensure appropriate to Oregon) or ability to practice per applicable regulations
- Case management, care coordination, or utilization review experience preferred
- Strong clinical assessment, critical thinking, and documentation skills
- Excellent communication and collaboration abilities
- Ability to work in a rural, critical access setting and maintain a 40-hour weekly schedule
- Nursing degree from an accredited program
